Learning Objectives By the end of this training, staff will be able to: Identify appropriate indicators for case closure and long-term stabilization Explain the importance of participant-centered and trauma-informed transition planning Demonstrate understanding of warm handoffs and community-based support connections Recognize risks associated with prolonged or stagnant cases Apply housing stabilization principles when evaluating ongoing service needs Distinguish between short-term stabilization services and long-term support systems Utilize supervision, case consultation, and escalation processes effectively Support clients in transitioning from services while maintaining dignity, transparency, and continuity of care Understand the role of accountability, documentation, and follow-through in successful case management outcomes Staff are encouraged to actively reflect on current cases throughout this training and consider how these principles can improve service delivery, client outcomes, and overall program effectiveness.
