Mysti's works alongside referral partners, housing providers, hospitals and clinics, reentry programs, employers, and community organizations across Washington. Use this page to refer a client or start a partnership conversation. Please do not include client names, ProviderOne IDs, diagnoses, or documents in this form. Our team will set up a secure exchange after first contact.

What Mysti's delivers in Washington

- Foundational Community Supports (FCS) — supportive housing and supported employment services for adults on Apple Health/Medicaid with complex needs. Enrollment runs through the state's FCS administrator (Wellpoint); FCS pays for support services, not rent.

- Community Choice Guide (CCG) — DSHS-contracted transition and setup help for people found eligible for Medicaid long-term services; referrals come through the person's HCS case manager.

- Case management and benefits navigation — helping people work every open door: Coordinated Entry, housing-authority waitlists, HEN/ABD, food, utilities, legal aid.

Any help with rent, move-in, or utility costs exists only inside a funded program the person is enrolled in, based on that program's eligibility and funding — it is never guaranteed, and we will tell you plainly when it is not available.
